      The most deaths are in sub-Saharan Africa. If you drew two lines across Africa, one below the Sahara desert (no malaria there because no mosquitoes!),and one above South Africa, 90%+ of the more than million deaths each year would be there. However, there are probably as many cases of malaria outside of Africa (India, SouthEast Asia, South America, Papua New Guinea – basically anywhere tropical). The difference in fatality rates is partly due to the intensity of transmission (people in Africa can get bitten with infected mosquitoes several times a day), and partly due to the different species of Plasmodium parasites (although there are four Plasmodium species that cause malaria, only one tends to be fatal, Plasmodium falciparum, and that one is most common in Africa).
